Our school will conduct the annual student flu vaccination on Friday, November 6, 2026.
To help us accurately prepare the required number of vaccine doses, all parents are requested to complete the online registration form, regardless of whether your child will be vaccinated at school.
Please review the following important instructions before filling out the form:
- Registration Period: From now until Tuesday, October 27, at 23:59. Please ensure you complete the registration before the deadline.
- Language Switch: If you prefer an English interface, click the globe icon in the top-right corner of the registration webpage.
- ID Requirement: Please enter your child's National ID number or ARC number (matching their NHI card). Do not use their school student ID. If you experience any login issues, please contact us via email.
- Vaccine Brand: The specific vaccine brand provided by health authorities cannot be determined in advance. If you have concerns regarding the brand, we recommend selecting "Decline" on the form and arranging for vaccination independently at a local clinic.
